Job Description and Responsibilities
Through business development, responsible for the initial patient identification, planning and coordination of admissions of total care patients to Valor Health Service and Valor Hospice Care. Supports the VHS Clinical and Patient Care team for continuity of care. For community outreach, supports Marketing Program and through business development, meets patient admissions goals.
· Identify, support coordination and planning with Intake/Customer Service all inquiries for admission to Valor Health Service’s Patient Care plan of care.
· Support admissions of patients, with coordination through the Nurse Practitioner and Practice Director and Administrator.
· Identify patient’s initial appropriateness for Valor Health Service’s services consistent with Valor Health Service’s Patient Care service policies and admission criteria.
· Serve as a Valor Health Service’s staff liaison with physicians, hospital/institutional nurses, social workers, discharge planners and community agencies.
· Supports implementation and coordination of services required for discharge of patients from hospitals into Valor Health Service’s Patient Care program of care.
· Visit patients and confer with hospital staff to ensure continuity and control of the plan of care.
· In accordance with policies and procedures ensure compliance with state and federal regulations.
· Provide information regarding Valor Health Service’s program of care and services to patients, physicians, and other sources of referrals.
· As part of Valor Health Service’s Continuous Quality Improvement Program, identifies need for creation or revision of policies and procedures, where applicable.
· Work with various community agencies and physicians to promote and facilitate the delivery of appropriate services for potential Valor Health Service patients.
· Participate in Valor Health Service’s community outreach/education efforts utilizing Valor Health Service’s staff and volunteers as appropriate.
· Working with the Practice Director and Administrator, supports actions plans and development of written information and promotional material as requested.
QUALIFICATIONS
- Minimum four years of varied work experience in a primary care, home care, or health care setting, preferred
- Demonstrated ability to relate effectively with patients, physicians, community agencies and institutions.
- Demonstrated ability to communicate effectively with members of the team.
- Demonstrated experience in education and training.
- Demonstrated excellent communication skills, written, and oral.
Job Type: Full-time
Pay: $48,000.00 - $52,000.00 per year
