A. Facilitates and coordinates appropriate patient admissions to the inpatient rehab unit.
Determines patients’ eligibility for admission through a comprehensive clinical assessment of key physical/functional, cognitive, and psychosocial metrics in order to determine patients ability to meet medical necessity/criteria for admission.
Criteria for admission to be assessed for every referral includes:
1. The patient requires the active and ongoing therapeutic intervention of multiple therapy disciplines (physical therapy, occupational therapy, speech-language pathology, or prosthetics/orthotics), one of which must be physical or occupational therapy.
2. The patient requires an intensive therapy program; under industry standard, this is usually three hours of therapy per day, at least five days per week; however, in certain, well-documented cases, this therapy might consist of at least fifteen hours of therapy within a seven consecutive day period, beginning with the day of admission to the IRF.
3. The patient requires physician supervision by a rehabilitation physician, defined as a licensed physician with specialized training and experience in inpatient rehabilitation. (The requirement for medical supervision means that the rehabilitation physician must conduct face-to-face visits with the patient at least 3 days per week throughout the patient’s stay in the IRF to assess the patient both medically and functionally, as well as to modify the course of treatment as needed to maximize the patient’s capacity to benefit from the rehabilitation process).
4. The patient can reasonably be expected to actively participate in, and benefit significantly from, the intensive rehabilitation therapy program.
B. Establishes and maintains the following:
a. Professional relationships with referral sources, patients, and family members to ensure the delivery of quality patient care.
b. Monthly marketing plans that strategically aligns with Baton Rouge Rehabilitation
products and service lines and overall goals and vision.
JOB FUNCTIONS
ESSENTIAL JOB FUNCTIONS include, but are not limited to:
1. Coordinates the referral process for patient admission to acute inpatient
rehabilitation as well as referral process for OP services.
PERFORMANCE STANDARDS:
- Evaluates and assesses referred patients to determine if patient meets medical
- Goal is to respond to a referral within 2 hours of receiving.
- Goal is to see every referral in person unless there are limiting factors such as access to facility or location of patient at time of referral.
- Determination of medical stability, readiness for program participation, and that services recommended are appropriate, reasonable and necessary. This is done in collaboration with the physical medicine and rehabilitation physician or other licensed treating physician with specialized training and experience in rehabilitation.
- Educate potential patients and families on acute rehabilitation and benefits of BRRH’s program(s). Distributes collateral materials appropriately.
- For inpatient referrals that meet criteria the clinical liaison will do the following:
- Complete a full preadmission screening (PAS) within 48 hours of patient admission.
- Provides payor source prior to admission to obtain admission approval to rehabilitation unit.
- Maintains accurate and precise log of patient referrals and referral outcomes for patients assigned to them and complete End of day documentation to be shared with the interdisciplinary team daily.
- Demonstrates knowledge of federal (Medicare), State and commercial payor regulations and guidelines regarding reimbursement of services.
- Recommends appropriate level of care and services for clients who are denied admission and providing guidance for clients who may benefit from rehabilitation admission in the future.
- Appropriately follows process to refer patients that do not meet criteria for inpatient rehabilitation to BRRH’s OP services.
PERFORMANCE STANDARDS:
- Creates target lists of contacts and decision makers in assigned hospitals. Develops professional relationships with targeted referral sources.
- Creates and maintains accurate monthly plans and reports for areas of responsibility and is prepared to review with one up at monthly meeting.
- Works with team and leader to identify opportunities for expansion of strategic business initiatives
- Regularly measures effectiveness of existing strategies and make adjustments to plans as needed.
- Supports marketing through educational programs, tours, meetings, etc. in assigned hospitals
- Develops strategies to grow market share in core business lines in assigned hospitals.
- Maintains and upgrades knowledge of BRRH products and services through defined marketing meetings and hospital/community training activities.
- Contributes to system planning efforts through mandatory participation in BRRH marketing meetings and annual retreats.
PERFORMANCE STANDARDS:
- Demonstrates the ability to educate staff, patients and physicians about all services that are afforded to patients at BRRH.
- Cultivating relationships with sources to generate repeat referrals.
- Participating in the facility's marketing education, ensuring knowledge of their resources, and maintaining knowledge of competitor facilities.
- Attending seminars and conferences to promote the facility.
- Acts as community resource and advocate regarding BRRH services.
- Maintains liaison relationships with contacts by providing information and education on rehab services, payer issues and related areas.

PERFORMANCE STANDARDS:
- Contributes to BRRH sales efforts through customer contacts, within agreed-upon timeframes.
- Promotes specialty clinics in community to highlight BRRH programs and services by identifying education needs of customers and organizing and implementing seminars as necessary.
EXPERIENCE REQUIREMENTS
Two years related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role preferred.
EDUCATIONAL REQUIREMENTS
Bachelor’s degree preferred.
SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Louisiana State licensure required as RN, LPN, PT, PTA, OT, COTA or ST.
CPR Certified.
Excellent communication and organizational skills. Ability to work alone and utilize effective time management skills. Proficient in spreadsheet, word processing and data processing.
Excellent clinical skill set/knowledge in order to appropriately and efficiently complete assessment of patients and determine medical necessity for admission to the inpatient unit.
