Work with a national leader in post-acute care as a Home Health Clinical Liaison, marketing their services to area hospitals, continuous care retirement communities, senior living, and physician offices.

You'll enjoy the flexibility to set your own schedule while focusing on goal achievement, patient care and expansion of referrals.

Duties for Home Health Clinical Liaison include:

• Establish and maintain contact with referral sources, hospitals, physicians and case managers
• Educate the medical community of clinical programs that improve patient outcomes and allow them to maintain their home environment and participate in on-going assessment of community needs.
• Participate in developing new business opportunities through community outreach and participate in on-going assessment of community needs.
• Present disease-specific clinical programs to promote positive patient outcomes.
• Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to home and assist in developing an agency-wide marketing plan.

Desired Qualifications for Home Health Clinical Liaison:

• RN preferred, but not required
• Sales experience in home health, hospice, home infusion, Oxygen or DME
