Community Liaison Manager

Responsibilities:

• Responsible for getting referral from local facilities and hospitals
• Being the primary contact person for community out-reach, promoting the company, and obtaining home care referrals of our non-medical home care services
• Responsible for intake calls, follow up and update leads status;
• Conduct in-home visits, initial assessments and re-assessments of prospects and clients; develop or adjust customized care plans accordingly;
• Work with CEO and other colleagues to develop referral sources and partners;
• Work with CEO and other colleagues to formulate company policy, procedure and practices in providing in-home care services.

Experience Required:

• Minimum Bachelor’s Degree OR experience and education as a substitute for minimum degree.
• Demonstrates knowledge of in-home care referral generation with measurable outcomes.
• Other skills include: analysis, counseling, knowledge of applicable laws, networking, organization, planning, and general marketing and sales

Pay: $58,240.00 - $75,000.00 per year
