Position Summary:
$10,000 Sign on Bonus available for Full time Position
The Hospital Liaison is to be accountable for census, business development, and sales-related activities for all designated assigned hospital accounts and additional external referral sources within the facility market. This is an external position that includes daily rounding activities with assigned hospital accounts, focused on census development, referral growth, and promoting efficient and effective transitions of care for skilled nursing facilities.
Core Responsibilities:
- Maintain high visibility in referring facilities, rounding with IDTs to identify patients who would benefit from LTAC placement.
- Collaborate with referring providers to determine optimal transfer timing based on clinical indicators.
- Develop strong relationships with key hospital personnel and identify new opportunities for alignment, partnering, and growth.
- Oversee the admissions process to ensure that referral/intake process is user friendly 24 hours per day; 7 days per week by working with hospital and designated facility staff.
- Maintain daily referral tracking log. Analyzes data for opportunities to expand services and markets and provides data and analysis to facility and corporate leadership team monthly.
- Assist in planning and organizing marketing strategies and programs designed to meet census and payor mix goals.
- Prepare monthly reports on sales activity, competitor/referral source changes in the marketplace, by completing a semi-annual competitive review.
- Assure that the admission process is seamless and positive, including but not limited to, identifying all necessary device/equipment and immediate medical needs necessary to ensure a smooth transition to the facilities.
- Understand and communicate critical and/or strategic changes to hospital functions, relationships, programs, and opportunities.
- Other duties as requested and assigned.
Minimum Qualifications:
· Respiratory Therapist, or RN/LPN with minimum 2 years ventilator experience.
- Familiarity with the admission criteria and process at LTAC.
- Experience with insurance verification including Medicare, Medicaid, commercial, and Managed care.
- Computer experience and understanding of hospitals EHR records preferred.
Job Type: Full-time
Pay: $85,000.00 - $105,000.00 per year
