Full description of the position

Summary

The Alliance Liaison enhances the continuity of patient care by providing collaboration between the hospital, physicians and hospice/home care, Home Medical Equipment services and the patients and caregivers. The incumbent works with case management, social workers, physicians and others on the health care team to transition the patient to the post-acute care setting. The Liaison will be responsible for building and maintaining partnerships that enhance the post-acute care referrals to Alliance. The incumbent collaborates with others to develop and execute market strategies to targeted audiences, improve service offerings and implement initiatives that support improved patient care and growth in division sales. REQUIRED:


Bachelor’s degree in a related field required.


Three (3) years of related clinical and/or marketing experience required.

Key Job Responsibilities

  • Participates as part of the Health Care Team to support a collaborative effort to facilitate quality patient care. Integrates and coordinates various aspects of service delivery, care facilitation, payer authorizations, service access, discharge and post-discharge activities and utilization review. Ensures documentation is complete by reviewing patient orders, verifying insurance information, gathering clinical data and determining patient care needs.
  • Identifies patients that meet criteria for Alliance Services. Arranges transfer of patients from hospital to home care/hospice services, consults with nursing, social services and other support services.
  • Promotes and builds strong relationships with referring physicians and hospital staff in the Case Management Department.
  • Provides information and education for health care team, physicians, hospital staff, and patient/family, answers questions and requests and completes applicable reporting as required. Effectively communicates in writing and orally with patients/families and other health team members.

  • Delivers and sets up portable medical and related equipment to necessary facilities as assigned with regard to the age specific populations including neonates, pediatrics, adolescents, adults and geriatrics. Assures equipment is operating safely and proper training is completed.
  • Implements initiatives to promote physician/hospital/Alliance collaborations, and strengthen new business alliances.

Physical Requirements

  • The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion.
  • Must have good balance and coordination.
  • The physical requirements of this position are: medium work - exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently.
  • The Associate is required to have close visual acuity including color, depth perception, and field of vision to perform an activity, such as assessing patients, preparing and analyzing data and figures; transcribing; viewing a computer terminal; or extensive reading.
  • Frequent invasive and non-invasive patient contact.
  • Exposure to patient body fluids as well as exposure to hazardous or poisonous materials.
  • Ability to react quickly to emergency situations.
