Audubon Home Health is a locally owned and operated full services home health agency that operates in Baton Rouge, Hammond, Lafayette and surrounding areas. Our mission at Audubon Home Health is to provide outstanding, high quality healthcare services to our patients and to referral sources within our coverage area.

We are currently recruiting for a dynamic LPN to join our team as a Clinical Liaison within a facility to act as a resource for all patients referred to home health by the physician. The Clinical Liaison actively coordinates transition of the patient from the facility to their home setting and provides feedback to providers, families and referral sources to ensure positive outcomes. The Clinical Liaison will also be an expert regarding the services provided by Audubon Home Health and will ensure effective communication and collaboration to providers.

Qualifications:

*One year of LPN experience in local market with knowledge of governmental home health regulations and Medicare requirements.

*Must have a current LA driver license and insured automobile in accordance with state standards.

*Must be licensed LPN in the state of practice.

Preferred:

*Prior sales/marketing experience in senior care and/or post acute care.

*Must function independently, have flexibility, personal integrity, and the ability to work well with others.
