Full description of the position

Work closely with clients and families, providing direct support for home care and community-based care coordination services. Conduct thorough professional assessments to understand clients' physical, emotional, social, and spiritual needs. Develop clear and stabilizing care plans, actively managing their implementation.

Be the face of our organization in the community by participating in local committees, task forces, and boards. Responsibilities include educating professional groups, raising awareness, and connecting with professionals to understand and address their needs collaboratively.

We are seeking a professional comfortable with older clients and their families, as well as with referral providers like social workers, attorneys and elite service professionals. The ideal candidate exudes confidence and approaches interactions with a genuine commitment to providing assistance.

Job Type: Full-time

Pay: $120,000.00 - $140,000.00 per year
